The form is provided to hospitals, outpatient facilities, and schools to help the student and school with a plan for the student returning back to school. The plan is filled out by the health care provider and given to the school nurse and guidance counselor. The purpose is to help make the transition back to school life as easy and simple as possible for both student and school.
